At least in some cases there really are underpants outside pants — I seem to remember a pirate in Balfonheim with that unusual strategy — but Baralai and Tatts (the fisherman npc all over the place in Spira) may be wearing traditional codpieces.
Sometimes I think Spiran society has collapsed so many times because of Sin that people just stopped caring about fashion. But ancient Zanarkand fashion is if possible even worse so maybe it's just always been like that.
I was going to say the Al Bhed have saner dress sense, but then I remembered their tenedency to wear mountain climbing harnesses everywhere, even underwater. LEG CHAFE.
It isn't just codpieces. There are a -lot- of Spira folk wearing exterior underwear [guys and gals both]. The mix of styles in Zanarkand especially is mind numbing: hot pants mixed with sewer worker garb, overalls and skimpy-wear everywhere! [...and yeah, I kinda like it.]
